Residential Services

Living Well Disability Services owns and operates 35 welcoming homes in 22 communities throughout the Twin Cities and surrounding counties for adults with intellectual and developmental disabilities. We currently offer Residential Services in six counties: Dakota, Hennepin, Isanti, Ramsey, Sherburne and Wright.  Our homes are centered around opportunities and choices – choices about everything from the colors of the bedspread and curtains, the flowers planted in the garden, and opportunities to try new activities in the community. Daily Living Supports include activities that enhance decision-making skills, develop sensory and motor skills,and strengthen interpersonal skills. Daytime support is provided for people who choose not to participate in outside employment, day training or habilitative services.

All of the daily living supports are designed to be person-centered and outcome based, helping individuals achieve their goals.

Health Services – Many adults with intellectual and developmental disabilities may face a wide range of physical health needs.  For individuals living in our homes, we provide nursing services, nutritional assessment & meal planning, occupational therapy assessments, environmental adaptations, assistive technology and communication devices, adaptive equipment, sensory integration and massage therapy.

Innovative wellness programming creates an opportunity for people to reduce their need for medication and increase their mobility. Results include a reduction in health related crises and reduced health care costs.

Recreational OpportunitiesJoy is a core value at Living Well Disability Services and everyone is encouraged to participate.  We provide many therapeutic and recreational programs.  Each home participates in our Garden Therapy Program, and individuals may participate in expanded wellness activities based on their personal interests.

Opportunities to be active in one’s own community through volunteering, taking classes, making new friends and maintaining relationships with family and friends are all part of a whole and active lifestyle. At Living Well Disability Services, we work hard to ensure everyone can choose their own level of activity and engagement.
