Living Well Disability Services

Experienced Direct Support Professionals help each person achieve the highest level of independence and quality of life possible through exceptional services.

Residential Services create a home environment based on individual choices about daily living, therapy, therapeutic recreation, health services and recreational opportunities. People served are able to complete daily tasks with dignity and independence through support from expert staff and investments in technology and therapy equipment.

Customized Services include In-Home Family Support (IHFS), Independent Living Services (ILS) and Semi-Independent Living Services (SILS). Support is planned through a specialized matching process to pair individuals with Direct Support Professionals that are uniquely qualified to meet their specific needs.

Innovative wellness programming creates an opportunity for people to reduce their need for medication and increase their mobility. Results include a reduction in health related crises and reduced health care costs.

Access to multiple community activities is made possible by safely operated and fully accessible vehicles. Outings are based on individual choice and examples include visiting family and friends, attending church and medical appointments, participating on sports teams, and working out at a health club.

Living Well Disability Services is able to provide exceptional quality of care to those served through the generosity of individuals, corporations and foundations that share a commitment to make a measurable difference in the lives of people impacted by disabilities.

Housing Transition Services support self-advocates and their families, guardians and case managers in pursuit of new options for affordable housing with appropriate support services. The services support increase choice and self-determination for people with developmental disabilities who wish to seek additional independence and self-direction.  A Living Well Disability Services Transition Coordinator provides in-depth information about housing choices, financial options and support programs to help individuals and their supporters navigate obstacles.
